KINDERGARTEN PRE-REGISTRATION

 

APRIL 2009

 

All children who expect to attend Kindergarten in Herrin Community Unit School District No. 4 next year will pre-register at the North Side Primary Center at 601 North 17th Street in Herrin on Friday, April 17, 2009.  Kindergarten classes meet for the full day.

 

Children may register as follows - if last name begins with:

A - F:            8:00 a.m.   -     10:00 a.m.

G - L:         10:00 a.m.   -     12:00 p.m.

M - R:        12:00 p.m.   -       2:00 p.m.

S - Z:            2:00 p.m.    -     4:00 p.m.

Registration will continue from 4:00 p.m. – 6:00 p.m.

for those families unable to attend at their designated time. 

 

Parents unable to attend pre-registration should call the North Side Primary Center at

942-5418 to report the name of the child.

 

A child must be 5 years old by September 1, 2009, to be eligible to attend Kindergarten during the 2009-2010 school year.  Parents should bring the child’s legal birth certificate (not the hospital footprint copy) to present for proof of birth date.  Parents should also bring the child’s Social Security card/number to present at the time of registration.

 

The Franklin-Williamson Bi-County Health Department nurses will be at the Kindergarten pre-registration.  At that time, they will be giving the vaccines required for your child to enter school.  The Bi-County Health Dept. charges $8.00 for each immunization.  The public aid medical card is accepted.  This is an excellent opportunity for you to get this taken care of, rather than waiting until summer or before school starts, when it becomes difficult to get appointments. However, they do ask that you bring a current immunization record card.  Your child will not be permitted to attend school this fall until compliance of the health laws are met.

 

The State of Illinois requires all kindergarten, second and sixth grade students to have a dental examination.  Dental examinations must be completed by May 15, 2010.

 

Those children presently enrolled in Kindergarten will not report for school on Friday, April 17, 2009.
